We know that every event, meeting, program and service at Beth Or is unique - that's what makes our community so vibrant and amazing! Please use the form below to submit all of your event details. If there is something you need but don't see in the form, please use the detailed description box to make your setup requests. 

Please submit this form 1 week prior to your event. Requests made after that may not be honored.

*Please choose either lemonade or iced tea
*Coffee and Tea setups include creamer, sugar, sweetener, etc
* If you have different beverage requests, please include them in your event description
*Beth Or is a Kosher Style facility, we do not allow for any pork or shellfish.
Audio Visual Setups:

*Please include a list of vendors, with their contact information. ie. caterer, photographer, dj, etc
*All vendors must have an up to date certificate of insurance on file
